Thank you for your post, Kendyl.
I agree about the many concerns about the nurse’s
behavior. I agree I would first make sure that all
required actions were done in the wake of the occurrence before checking in
with the patient, the provider, and the nurses. I’d like to address the
mistake with the two nurses and arrange for an in-service to remind the nursing
team of the medicine administration procedure. The nurse’s past and behavior
after the occurrence would determine the nature of the disciplinary penalty.
This could be an opportunity for instruction or a sign that something needs to
be written up or dismissed.
THIS IS THE POST I AM RESPONDING TO:
Professional accountability is a crucial ethical principle to have
as a nurse. Having accountability is taking responsibility for one’s own
actions and their outcome. Nurses have to be honest and hold themselves to a
high standard when providing care to patients as they make life changing
decisions every day. Even in the small moments that seem insignificant, it is
important for the nurse to uphold this standard (Luggar-Schmit, 2024). In this scenario, nurse Lope
accepts these medications from nurse Jim and blindly trusts that they are okay
and gives them to a patient, even when the MAR showed that they were not due at
this time and not the correct medications for the patient. Nurse Jim left
unfinished work for the next shift and handed off medications that were his
responsibility to give. He was dishonest about the time frame that he was
supposed to have given them in and he was giving incorrect medications. If I were
nurse Jim, I would have either given or withheld the medications myself after
doing my correct medication checks and verifying with the MD that they were
okay to give late. If I were nurse Lope, I would refuse to give the medications
handed to me by Jim unless they were verified and within the time frame. I
would ask that Jim and I review the MAR together before he leaves and check the
medications prior to giving them. I would have notified the physician of the
medication error and followed up with the patient. If I still somehow managed
to give the medications at the wrong time and to the wrong patient, I would
report the mistake according to hospital policy and monitor the patient’s
condition.  
There are
many concerns about the nurse’s behaviors I would address as the nurse manager.
Both nurses did not follow safe standards of care and ultimately the patient
was given incorrect medication. I would first ensure that all the necessary
steps were taken following the incident and follow up with the provider,
patient, and nurses. I would calmly discuss the error with both nurses and
provide an in service for the nursing staff to attend to remind them of
protocol with providing medications. The disciplinary action would be based on
the nurse’s actions following the incident and their history. This could be a
teaching moment or a moment that indicates a need for a write up or dismissal.
It is important to provide an environment that encourages staff to report
errors without fear of losing their job or license. This can improve patient
safety and trust within the workplace (Nursing Center, 2019).
